Save time when you come in by filling out our new patient form. You can download a PDF Version of this form. Print it out and complete from the convience of your home. You are also welcome to complete the online version of this form and it will be sent directly to us.
Step 3: Attend Appointment

Our Snowflake Location
Arrival Time: Please arrive 15 minutes before your appointment so we can verify your information. Upon arrival all patients must complete New Patient paperwork (if not already completed), provide insurance cards and photo ID.
Appointment Duration: Appointments will depend on complexity of your case. You should plan for 30 minutes to 1 hour.
What To Expect: During the appointment our podiatrists' will complete a thorough evaluation which may include an X-ray and/or Ultrasound. Depending on the customer need care may be provided on the initial visit.
